Baylor Scott & White Community Care – Carrollton is devoted to the total health care of uninsured and under served people of Carrollton and its surrounding communities. We provide continuing medical care for individuals and their family members of all ages, from children 5 and older to the elderly.
Our expertise is based on the knowledge of our patients in the context of their community. This community has a very high incidence of uninsured residents, many of which suffer from chronic diseases such as diabetes. Our primary focus is on providing a medical home to those residents that are in need in order to assist in their overall health and wellness.

Baylor Scott & White Community Care – Carrollton offers comprehensive care for patients of all ages. Our physicians and staff are able to provide a variety of health services allowing patients to receive all of their health care needs within our office.
We welcome new patients and can often accommodate same day appointments.
Making an Appointment
On your first visit, we will begin a permanent record. Please bring any medications you are taking, so that we may accurately determine the dosage and frequency. We will ask you to complete a medical history and registration form. Please bring your insurance card and try to arrive at least thirty minutes early for your first visit. If you have been under the care of another doctor, we may ask your permission to transfer some of your records.
We have three appointment categories that take up varying amounts of time:
